Sliding-Scale Policy
• 25–30% of total AYB Clubhouse™ enrollment reserved for sliding-scale or reduced-fee clients
• Eligibility determined by household income (federal poverty level guidelines) and financial hardship documentation
• Application reviewed confidentially by Admissions; decision within
• Families reapply each quarter with updated income verification
• All applications handled with full confidentiality and dignity

IPC / Plan of Care (POC) Signing Requirements
• Guardian and youth (age-appropriate) must sign the IPC/POC within7–14 days of authorization
• Signing acknowledges acceptance of the therapeutic respite model, 3-month outcomes, MTS/Health Home integration, and out-of-pocket responsibilities
• Digital signatures accepted via a secureHIPAA-compliant platform
• Failure to sign within the timeline may delay services or result in waitlist placement
• All fees due at the start of each 3-month authorization period
